### 1.2 Structural Description

Tesamorelin is a synthetic 44-amino-acid analog of endogenous human growth hormone-releasing hormone (GHRH). It corresponds to the full-length human GHRH(1-44)-NH₂ sequence with a key structural modification: the N-terminal tyrosine residue is modified with a *trans*-3-hexenoyl group. This N-terminal acylation confers resistance to enzymatic degradation by dipeptidyl peptidase-4 (DPP-4), significantly extending the peptide's in vivo half-life compared to native GHRH.

### 1.3 Mechanism of Action

Tesamorelin acts as a selective agonist of the growth hormone-releasing hormone (GHRH) receptor (GHRHR) on pituitary somatotroph cells. Upon receptor binding, it stimulates the synthesis and pulsatile secretion of endogenous growth hormone (GH), which in turn stimulates hepatic production of insulin-like growth factor 1 (IGF-1). This GH/IGF-1 axis activation reproduces the physiological effects of endogenous GHRH.

Unlike recombinant human GH (rhGH) replacement therapy, Tesamorelin preserves the endogenous feedback regulation of the GH axis, including the negative feedback modulation by IGF-1 and somatostatin.
